Quit Smoking Services

Quit Smoking Support Services are offered by health promoters at both sites in Fort Erie and Port Colborne/Wainfleet and open to anyone living in these communities and interesting in quitting smoking cigarettes. 

How to Access:  

  • Bridges CHC primary care clients can ask their health care provider to make an internal referral for individual counselling. 
  • Residents of Fort Erie, Port Colborne, or Wainfleet can ask their family doctor/nurse practitioner to complete this referral form: Professional Counselling Referral Form, and fax to Bridges CHC. 

What to Expect: 

Eligible participants can create a quit plan with a trained smoking cessation counselor and receive free nicotine replacement therapy to support their quit. Contact Bridges CHC for more information.
