
Primary Care
Your health care at Bridges CHC is supported by a range of health professionals. Clients are rostered to either a physician or nurse practitioner with Bridges CHC who will oversee their primary health care. However, clients may have appointments with other health professionals to support their care.
As a primary care client of Bridges CHC, your health care team may include appointments with a:
- Physician
- Nurse Practitioner
- Registered Nurse
*Bridges CHC practices a team-based approach to primary care appointments. Depending on the purpose of your visit, you may be seen by the nurse, nurse practitioner, or physician. You will be booked for an appointment with the most appropriate provider based on your illness, health concern, or reason for appointment.

- Dental Health Worker- support to access publicly funded dental health programs including the Ontario Seniors Dental Care Program and the Canadian Dental Care Plan, find more information here. (outreach/navigation pg)

- Health Promoter- to co-create a quit plan and if eligible, access free nicotine replacement therapy, find more information here (quit smoking pg)
