
Physiotherapy Program
Bridges Community Health Centre offers free physiotherapy services to residents of the community who do not have private insurance or a workplace benefit plan.
The program offers an individualized, patient-centered assessment and plan to treat physical conditions that would benefit from physiotherapy. Clients will be supported to manage their condition independently with education, home-exercise program, and referral to other community programs.
Who can access Physiotherapy: You do not have to be a patient of Bridges CHC to access the Physiotherapy Program. Anyone who lives in communities of Fort Erie, Port Colborne, or Wainfleet can access individual appointments or group exercise classes. *If you have private insurance or workplace benefits, we require they be used first.
How to access Physiotherapy Services: You can refer yourself or be referred by a doctor or nurse practitioner using the intake form here: Physiotherapy Program Intake Form
GROUP EXERCISE CLASSES
Exercise classes are open to the entire community at no cost to promote cardiovascular health, strength, and improve stability. If you are interested in attending one of the exercise classes, in addition to registering for the class of your choice, please complete this form prior to participating in your first class: Exercise Program Intake Form and Waiver
