Outreach/Navigation Services 

Community Health Workers: 

For people who need support getting connected with health service and programs at Bridges CHC or other supports and resources that are available in the broader community a Community Health Worker can help by:   

• Offering non-judgemental, confidential support 

• Informing you of available services and resources in our community 

• Referring you to services and advocating that you receive the help you need 

• Assisting you with form filling and attending appointments 

• Offering volunteer opportunities within our Community Health Centre and informing you of other ways to get involved and make a difference 

Who Can Access System Navigation Services:? 

Nutrition counselling is available to community members living in Fort Erie, Port Colborne, or Wainfleet and who do not have private insurance or a workplace benefit plan. Time limited sessions with a Registered Dietitian will provide clients with the opportunity to learn more about Canada’s food guide, your relationship with food, and the potential of food to support your mental and physical health.

Dental Health Worker: 

ONTARIO SENIORS DENTAL CARE PROGRAM 

You may be eligible for the program if you:  

· are 65 years of age or older; 

· are a resident of Ontario; 

· as of August 1, 2021 have an annual net income of $25,000 or less for a single person, or a combined annual net income of $41,500 or less for a couple; 
(line 236 on an income tax form) 

· have no other dental benefits. 

CANADIAN DENTAL CARE PLAN  

The Canadian Dental Care Plan (CDCP) will help ease financial barriers to accessing oral health care for eligible Canadian residents. Applications will open in phases starting with seniors. Find out more about the Canadian Dental Care Plan here.
